Fallon Health’s innovative Summit ElderCare® program celebrates 10 years on Grafton Street

Worcester, Mass., December 19, 2018 —Fallon Health, a nationally recognized, not-for-profit health care services organization, celebrated a decade of providing care and services to seniors at its Summit ElderCare site on Grafton Street site in Worcester.

Fallon’s innovative Summit ElderCare program, first launched in 1995, is a national model of care known as a Program of All-Inclusive Care for the Elderly (PACE), which provides participants with an array of coordinated, integrated care and support that allows them to remain living at home. Summit ElderCare is available to any individual who is 55 years or older and eligible for nursing home-level care.

The Grafton Street site’s 10th anniversary was marked by a ceremony where employees, members and their caregivers were recognized and appreciated for their participation in the ground-breaking program, which opened in 2008.

“We are celebrating a successful partnership between Fallon and the wonderful people of this community,” said Richard Burke, President and CEO of Fallon Health. “Everyone involved deserves credit for building a program that offers a compassionate approach to caring for our loved ones.”

During the ceremony, state Rep. Mary Keefe (D-Worcester) presented Fallon with a citation from the House of Representatives recognizing the milestone anniversary and the program’s service to the community.

Annamaria Salisbury, Executive Director of Summit ElderCare, presented flowers to individuals who have worked at the Grafton Street location for all 10 years: Nancy Grigas, Supervisor of Recreational Activities; Mary Jeanne Duhamel, Secretary/Receptionist; Bernadette Peloquin, Occupational Therapy Assistant; Leonora Masiga, Health Aide; Nancy Saal, Dietitian; and Sharon Perno, Physical Therapy Assistant.

Today, more than 1,100 seniors are cared for at Fallon’s Summit ElderCare centers located in Leominster, Lowell, Springfield, Webster and Worcester. Fallon Health also runs a PACE program in Amherst, New York, in partnership with the Weinberg Campus.
