A plan for people who qualify for financial help to pay for health insurance (subsidy)

What is Community Care?

Community Care is an affordable health plan in central Massachusetts and parts of Middlesex County that includes Reliant Medical Group (including Southboro Medical Group), Harrington HealthCare, select PCPs and specialists affiliated with MetroWest HealthCare Alliance and other local providers.

You'll get reduced premiums, low copayments, no deductibles and a $150 annual fitness reimbursement.

Am I eligible?

If you live in central Massachusetts or parts of Middlesex County and qualify to get financial help to pay for health insurance, you’re eligible for Community Care. Don’t think you qualify for financial help? You might be surprised.

1 Between $15,800 and $35,640
2 Between $21,307 and $48,060
3 Between $26,813 and $60,480
4 Between $32,319 and $72,900
5 Between $37,825 and $85,320
6 Between $43,331 and $97,740

What do I get with Community Care?

You’ll pay reduced monthly premiums, have low copayments and cost sharing, and you’ll have no deductibles for covered medical services and prescription drugs. Plus, you’ll get extras like:

  • The most flexible fitness benefit available in Massachusetts, reimbursing up to $150 annually for participating in healthy activities such as school and town sports, gym memberships, dance and fitness classes, home fitness equipment and much more.

  • Teladoc™ telehealth—members get 24/7 access to a national network of U.S. board-certified doctors to discuss non-emergency conditions by phone, mobile device or online.

  • Key doctors and hospitals within central Massachusetts and parts of Middlesex County—including Reliant Medical Group (including Southboro Medical Group), Harrington HealthCare System, select PCPs and specialists affiliated with MetroWest HealthCare Alliance, Saint Vincent Hospital, Clinton Hospital, HealthAlliance Hospitals, Marlborough Hospital, MetroWest Medical Centers and Milford Hospital.
