Fallon Community Health Plan opens Summit ElderCare® in Springfield

The first PACE program available to Western Mass. seniors

Springfield, Mass., July 29, 2013 ― Fallon Community Health Plan (FCHP), one of the country’s leading providers of coordinated care for seniors, announced today the opening of a new senior care facility, Summit ElderCare®, a Program of All-inclusive Care for the Elderly (PACE), in Springfield. FCHP is the first to bring this nationally-recognized model of care to the Western Massachusetts region.

Summit ElderCare offers a community-based alternative to nursing home care for older adults who want to avoid moving into a nursing home. Based on a national model of care and coverage recognized by both Medicare and Medicaid, Summit ElderCare provides medical care, geriatric case management, care coordination, adult day health services, full insurance coverage, including Medicare Part D prescription coverage, and in-home support—all in one personalized program.

Recognizing that nearly one in three Americans act as a caregiver to a spouse, older relative or friend*, Summit ElderCare also offers solutions and support for the caregiver as well.

One of the hallmarks of the Summit ElderCare PACE program is the interdisciplinary team which includes a range of professionals such as a primary care physician, nurses, a nutritionist, a transportation coordinator, a home care coordinator, a social worker and others. “In addition to providing specialized geriatric care, Summit ElderCare’s interdisciplinary team surrounds participants and their families with a wide array of supportive services that help loved ones to continue living in the community,” said Dr. David Wilner, Vice President and Medical Director of Summit ElderCare.

The Summit ElderCare program is open to any person age 55 or older residing in the service area of Hampden County as well as parts of Hampshire County, Southampton, Easthampton, South Hadley and Granby. The program also serves residents of Worcester County as well as the communities of Hudson and Marlborough. To be eligible for enrollment, individuals must require a nursing home level of care and be capable of living safely in the community with Summit ElderCare support.

Summit ElderCare’s new 14,500-square foot PACE center is located in Springfield’s North Medical District on 101 Wason Avenue, easily accessible to Route 91. “It has been a pleasure to work with folks in Hampden County,” said Karen Longo, Vice President and Executive Director of Summit ElderCare. “The Springfield community has been very welcoming.”
