Summit ElderCare helps older adults live at home even though they qualify for nursing home care. With Summit ElderCare, you get complete medical care and social support tailored to your loved one's needs. The care team arranges all services, and you get access to geriatric care experts, adult day health centers, home care and more.**

* If you have MassHealth Standard, but you do not have Medicare Parts A and B, you may be eligible to enroll in our NaviCare SCO program. If you have Medicare Parts A and B in addition to MassHealth Standard, you may enroll in NaviCare HMO SNP.

** In some cases, a monthly premium may be required based on the participant's income or assets. All services must be provided or authorized by the PACE interdisciplinary team, except emergency services. PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.

Fallon Health is an HMO/HMO-POS plan with a Medicare contract and a contract with the Massachusetts Medicaid program. Enrollment in Fallon Health depends on contract renewal. NaviCare is a voluntary program in association with MassHealth/EOHHS and CMS.
