Getting organized

A list of necessary information related to the person for whom you care is essential. Stress can make even the simplest things to remember the easiest to forget. Having everything written down is great for everyone involved in your loved one’s care.

Include on this list:

  • Doctors' names and numbers
  • A list of prescription and over-the-counter medicines, herbal supplements and vitamins (along with the dosage, who prescribed it and why, and any side effects
  • Allergies to medicines, foods or insects
  • Symptoms, health conditions, and treatments
  • Emergency medical information, such as pacemaker use or chronic disease diagnosis.
  • Insurance policy numbers
