Fallon Health’s NaviCare® program expands services with South Cove Community Health Center
Worcester, Mass., July 12, 2018 — Fallon Health, a nationally recognized, not-for-profit health care services organization, announces that members of its statewide NaviCare program—a Medicare Advantage Special Needs Plan and Senior Care Options program—now have access to South Cove Community Health Center in Boston, Quincy and Malden.
South Cove Community Health Center, an affiliate of Beth Israel Deaconess Medical Center, is the largest Asian community health center in Massachusetts. Since 1972, South Cove Community Health Center has brought care and health services to many underserved residents of the greater Boston community.
Fallon Health’s NaviCare program integrates care for adults age 65 and older. A personalized care team manages and coordinates the NaviCare member’s health care by working with each member, the member’s family and health care providers. The goal is to ensure that each member remains living as independently and safely as possible in the community.
“We’ve seen the NaviCare program improve the health and well-being of thousands of older adults throughout Massachusetts,” said Deb Daviau, Fallon Health’s Executive Director of its NaviCare program. “Our partnership with South Cove Community Health Center is another step we’re taking to ensure that a high level of care and personalized service is available to more residents in the Quincy region.”
South Cove offers services with health care professionals who are fluent in Chinese (Mandarin, Cantonese, Toisanese, Taiwanese) and Vietnamese. Services include, but are not limited to, adult medicine, obstetrics/gynecology and specialties such as cardiology, neurology and podiatry.
