Fallon Health names Mary Ann Graham as Site Director of its Summit ElderCare program in Worcester
Worcester, Mass., July 24, 2018 — Fallon Health, a nationally recognized, not-for-profit health care services organization, is pleased to announce the recent hire of Mary Ann Graham as Site Director of its Summit ElderCare® location on East Mountain Street in Worcester.
In her role as Site Director, Graham will oversee the daily operations of Fallon’s Summit ElderCare program on East Mountain Street—a Program of All-inclusive Care for the Elderly (PACE)— while interacting with participants and caregivers.
Graham will lead an interdisciplinary team that includes a range of professionals who specialize in geriatric care, including primary care physicians, nurses, nutritionists, transportation coordinators, home care coordinators, social workers, and rehabilitation therapists, ensuring that each participant’s personalized care plan allows him or her to live as independently as possible.
Prior to her role as Site Director, Graham was Manager of Quality and Compliance at Cambridge Health Alliance Elder Service Plan. She also was a dietitian and Elder Care Nutrition Program Lead at Reliant Medical Group, where she worked for 17 years, several of them serving Fallon’s Summit ElderCare program.
In addition to receiving a Bachelor of Science from Framingham State College, Graham earned a Master of Science in nutrition as well as a Master of Science in Management of Aging Services from the University of Massachusetts, Boston.
Fallon’s Summit ElderCare program has six sites in Massachusetts: Leominster, Lowell, Springfield, Webster and Worcester (two locations). Fallon also operates a PACE program, Fallon Health Weinberg-PACE, in western New York. Fallon Health is the largest provider of PACE in New England and the fifth largest in the country.
