Plans and benefits

With a choice of four FCHP plans, you not only get to choose a level of coverage that fits your needs, you also get to choose your network of doctors and hospitals. No matter which FCHP plan you choose, you’ll get:

• $0 routine physical exams and well-child visits
• Yearly eye exams and discounts on eyeglasses and contact lenses
• Up to $300 reimbursement ($300 for two-person or family contract/$150 for individual contract) for eyeglasses and contact lenses purchased at Fallon Clinic Optical Centers
• Prescription coverage around the country
• Emergency care around the world
• Up to $400 reimbursement ($400 for two-person or family contract/$200 for individual contract) for fitness center memberships and school and town sports programs
• A variety of health education and wellness programs, such as smoking cessation and more!

Provider and pharmacy networks

• The FCHP Direct Care network includes providers at Fallon Clinic, Acton Medical Associates, Charles River Medical Associates, Southboro Medical Group, Lawrence General Hospital and Independent Practice Association, Lowell General Physician Hospital Organization, and Northeast Physician Hospital Organization. The FCHP Direct Care network also includes the following hospitals:  Emerson Hospital, Harrington Memorial Hospital, HealthAlliance Hospitals, Hubbard Regional Hospital, MetroWest Medical Center, Milford Regional Medical Center and Saint Vincent Hospital.
  
  
• The FCHP Select Care network features more than 13,000 providers and more than 40 hospitals throughout Massachusetts. Here are just some of the providers in the FCHP Select Care network: Fallon Clinic, Acton Medical Associates, Baystate Health, Charles River Medical Associates, Children’s Hospital, Emerson Hospital, Greater Milford Health Alliance, Harrington Memorial Hospital, HealthAlliance Hospitals, Heywood Hospital, MetroWest Medical Center, Lahey Clinic, Lawrence General IPA, Lowell General Hospital, Milford Regional Medical Group, Mount Auburn Cambridge IPA, Nashoba Valley Medical Center, Newton-Wellesley Hospital, Saints Memorial Medical Group, Southboro Medical Group, Tri-County Medical Associates, Tri-River Family Physicians and UMass Memorial Health Care System.
  
  
• Fallon Preferred Care is a PPO product. Members have access to a national and regional network, plus the choice to see nonparticipating providers (at the out-of-network level of coverage).
  
  
• Pharmacy network - With FCHP, you can have your prescriptions filled at any of their thousands of affiliated retail pharmacies nationwide, in addition to many community pharmacies and the PharmaCare pharmacies located in Fallon Medical Centers. You also can fill prescriptions at the following major retail chains: CVS, Brooks/Rite-Aid, Walgreens, Target, Kmart, Wal-Mart, Shaw’s and Stop & Shop.
