Fallon Community Health Plan appoints Patricia H. Forts to Senior Director of Strategic Planning

Worcester, Mass., November 25, 2008― Fallon Community Health Plan (FCHP), a nationally recognized, not-for-profit health care services organization, is pleased to announce the appointment of Patricia H. Forts to Senior Director of Strategic Planning. In this position, Forts will lead many of the key initiatives critical to FCHP’s growth and diversification strategy, including service area expansion, corporate planning and business development for both the Health Plan Operations and Senior Care Services divisions.

Forts has held several key positions since coming to FCHP in 2004, including Director of Compensation and Benefits and Director of Marketing Communications. Most recently she served as Director of Strategic Planning. She came to FCHP from Fallon Clinic where she held numerous management positions in Human Resources.

Forts graduated cum laude from the College of the Holy Cross, where she received a bachelor’s degree in sociology. She is presently a member of the Society for Human Resources Management and the Shrewsbury High School Council, and is a Disaster Action Team Member for the American Red Cross.

She resides in Shrewsbury with her husband, Jeff, and their two children.
