Essential Piece Award
The Essential Piece Award recognizes an individual whose skills and efforts made a difference and resulted in a successful outcome for FCHP.

Beyond the Call Award
The Beyond the Call Award recognizes an individual who is an advocate for our members and delights our customers with dedicated customer service.

Employee Achievement Award
The Employee Achievement Award is  given to an individual who makes ongoing, outstanding contributions every day. This employee is known for having achieved exceptional performance by demonstrating commitment to customer service, quality and innovation.

Summit ElderCare Peak Performer Award
The Peak Performer Award is given to an employee at Summit ElderCare^® who exhibits outstanding performance and dedication.

Above and Beyond
Through the Above and Beyond program, managers and directors can recognize individual or team efforts that exemplify FCHP’s vision, embrace our values and go above and beyond the normal scope of the job at any time during the year. Recipients may have collaborated with other departments to improve processes and solve problems, created and implemented a process that saves time and money, or actively promoted our mission and values in the community.

F.E.S.T. Award
The F.E.S.T. Award can be presented for a variety of reasons: partnering successfully, meeting or exceeding customer’s needs, turning a dissatisfied customer into a satisfied customer, taking the initiative to go beyond what is expected or required, and/or when an employee’s efforts result in a significant benefit and measurable contribution to the organization.

Leading by Example
The Leading by Example award is  given to a manager who demonstrates the values that FCHP stands for in his or her every day work. The recipient is a true leader in our organization and guides the way with great tact and dependability. He or she motivates others to perform at their best, leading the entire team toward ultimate success.

Together We Succeed Award
The Together We Succeed Award is a biannual award and recognizes a “team” of employees who work together regularly or as part of a project who achieved results towards a corporate goal.

Outstanding Achievement Award
The Outstanding Achievement Award is given to a team of employees who have demonstrated outstanding achievement in reaching business goals.
