All other FCHP plan members
Our mail-order pharmacy offers four ways to order prescription refills:
- Online through the Caremark's Web site. (On the Caremark Web site, you can also view your prescription history, research generic alternatives, look up copayment information and find a local pharmacy.)
- By phone: access the automated refill system by calling 1-888-543-5919 (toll-free)
- By mail: included with each order will be a pre-addressed, postage-paid envelope which can be used to refill or order a new prescription
- By fax: physicians can fax in new or refill orders to this fax number: 1-800-378-0323
Your prescriptions will be delivered free to your home within 10 to 14 days from when our mail-order pharmacy receives your prescription or refill request.
Placing an order for the first time
To place your first order, you’ll need to complete a Mail Service Enrollment Form for yourself and your covered family members. Simply mail the form or submit the information online and send the original prescription to CVS Caremark Mail Service Pharmacy. You can pay for your prescriptions by credit card and, for orders you submit by mail, with a check or money order.

What if I have questions or I don't get my prescription in time?
If you have any questions about your prescription order, or don’t receive your prescription within 14 days, call the CVS Caremark Mail Service Pharmacy toll-free at 1-888-543-5919 (TDD/TTY: 1-800-238-0756), Monday through Friday 8 a.m. to 10 p.m., Saturday 9 a.m. to 6 p.m. and Sunday 10 a.m. to 6 p.m. Eastern time. You may also call FCHP's Customer Service at 1-800-868-5200 (TDD/TTY: 1-866-608-7677). Monday through Friday, 8 a.m. to 6 p.m.
