Enroll in one of our Medicare care plans

Follow these three easy steps to enroll:

1. Download and print the instructions and forms for the plan you'd like to join:

     Download this document if you're interested in the Fallon Seinor Plan Saver plans, Fallon Senior Plan Standard plans, or Fallon Senior Plan Plus plans.
     
   • Enrollment instructions and form for our PPO plan (Fallon Senior Plan Preferred Enhanced Rx)
     
   • Enrollment instructions and form for our Special Needs Plans
     Download this document if you're enrolled in both Medicare and Medicaid/a Medicare Savings Program and you're interested in Fallon Senior Plan Value 1 or Fallon Senior Plan Value.
     
2. Make a copy of your completed application for your records.
   
3. Get your completed application to us by mail, by fax or in person.
