Pharmacy finder look-up
With this pharmacy finder look-up tool, you can look up more than 24,500 pharmacies that are included in our Fallon Senior Plan pharmacy network. FCHP contracts with pharmacies that equals or exceeds the Centers for Medicare & Medicaid Services requirements for pharmacy access in your area.
If you get your health insurance through your or your spouse’s current or former employer, contact the benefits administrator. Your pharmacy network may be different than the pharmacies than are listed here.
To find a pharmacy:
- Click on “View the entire list” to view all of the pharmacies in our network*
or
- From the drop down menu, select the state in which you would like to look up a pharmacy. Then, click on the letter that corresponds to the first letter of the name of a pharmacy’s name.
