What is Senior Care Services?
Our Senior Care Services division oversees all products and solutions which focus on the care of our senior population. Those solutions include:
Medicare products
Our Fallon Senior Plan Medicare Advantage offerings for Medicare-eligible individuals include HMO and PPO plans. We also offer two different levels of Medicare prescription drug coverage, as well as plans without drug coverage. Our wide range of plans with different premiums and levels of benefits allows seniors to choose the plan that meets their needs and their budget.

Summit ElderCare®
FCHP sponsors Summit ElderCare, a Program of All-Inclusive Care for the Elderly (PACE). Summit ElderCare is a complete medical, insurance and social support program for elderly people who qualify for nursing home care. Our team of geriatric social workers, nutritionists, rehab specialists, home health care staff, and doctors and nurses oversee your loved one’s care at home and at our adult day health centers. Summit ElderCare's services also include prescription drugs and medical transportation plus resources for caregivers.

NaviCaresm HMO
NaviCare HMO is a new Senior Care Options product which provides coordinated medical, prescription and support care for seniors who are 65 or older, live in Worcester County, and are eligible for MassHealth Standard. NaviCare HMO provides services covered by Medicare and MassHealth, plus other benefits that the member may need.

Through its Summit Living, Inc. branch, Senior Care Services is committed to developing residential solutions for our community's senior population. We will provide independent, assisted, and memory care living options for the communities we serve.
Home Staff, LLC
Home Staff, LLC, a joint venture with VNA Care Network & Hospice, has offered private home health care with integrity, respect and compassion since 1977. Its well-trained and experienced aides care for individuals requiring personal care, recovering from surgery, receiving hospice care, suffering from Alzheimer’s, and/or needing household management, among other services.
