What is NaviCare HMO?
NaviCare HMO is a health care program sponsored by Fallon Community Health Plan. NaviCare HMO helps you get the most out of your Medicare and MassHealth benefits and offers you extra services that can help you stay healthy at no extra cost.
- Your primary care physician will work with a team to develop your own personal care plan based on what kind of care you need. You'll also have your own personal Nurse Case Manager and Navigator who will work together to coordinate and manage your health care needs along with your doctors, social workers and other health care specialists.
- With NaviCare HMO, you'll have no premiums and no copayments.
- NaviCare HMO provides all of your medical and prescription drug needs, as well as home and personal care services.
Who is eligible?
You're eligible for NaviCare HMO if you:
- Are 65 or older
- Have both Medicare and MassHealth Standard or
MassHealth Standard alone
- Live at home or reside in a long-term health care facility
- Live in Worcester County

Enrollment in NaviCare HMO is voluntary. This program is available through a cooperative agreement with the U.S. Department of Health and Human Services, the Centers for Medicare & Medicaid Services and the Commonwealth of Massachusetts’ MassHealth Program.
FCHP is a health plan with a Medicare Advantage contract. Medicare Advantage contracts between the federal government and managed care organizations are valid for one calendar year. The benefits, premiums, copayments and service areas offered by FCHP are subject to change on an annual basis
