Caremark ExtraCare Health Card® discount
A new discount program for Direct Care, Select Care, Preferred Care, and Fallon Senior Plan members!
Effective January 15, 2010, all eligible members will receive a CVS Caremark ExtraCare Health Card which allows you to receive a 20% discount on certain items at any CVS/pharmacy® store or online at www.cvs.com.
- 20% discount applies to regularly priced CVS/pharmacy Brand or CVS/pharmacy Exclusive Brand health-related, flexible spending account (“FSA”)-eligible items of $1 or more.
- FSA-eligible items include more than 1,500 CVS/pharmacy Brand products such as pain relief, allergy relief, and cough and cold remedies as well as products like glucose meters and blood pressure monitors.
- You can use your new CVS Caremark ExtraCare Health Card on FSA-eligible items even if you are not enrolled in a Flexible Spending Account.
Eligible FCHP members will receive a letter explaining the discount and three portable CVS Caremark ExtraCare Health Card key tags in the mail. The key tags can be used by spouses and dependents too. The CVS Caremark ExtraCare Health Card will replace any existing CVS/pharmacy ExtraCare Card that you may already have. You'll continue to receive all the benefits you have today with your ExtraCare Card in addition to the benefits provided with the ExtraCare Health Card.
Important note:
FCHP MassHealth members, NaviCare HMO members, and Summit ElderCare participants are not eligible for the CVS Caremark ExtraCare Health Card discount at this time.
Home health agency discount

Dental discounts
Apart from any preventive dental benefits you may receive, members also receive 25% to 50% discount on services from general dentists and a 20% discount on services from dental specialists. (Not available to FCHP MassHealth members.)
Eyewear discounts
FCHP members receive discounts on frames, lenses, contact lens packages and nonprescription sunglasses when purchased at participating optical shops. Discounts vary. Ask your optician and/or optical shop to see what services discounts they offer to Fallon Community Health Plan members. (Not available to FCHP MassHealth members.)
Fitness reimbursements
Through FCHP's It Fits! program, members receive reimbursement ($400 per family contract/$200 per individual contract) for:
- Health club memberships
- Weight Watchers®
- Local town and school sports programs
- Aerobics, Pilates and yoga classes
