Employer Tools registration

Registering for Employer Tools is as easy as 1-2-3! Complete just one registration form to access all of the tools.

Step 1:
Download the registration packet, which includes the Employer Tools Enrollment Form and Billing Agency Authorization Form.

Step 2:
Complete the requested information.

Step 3:
Mail the registration information to:

Upon receipt, FCHP will verify your status as an employer and assign you a username and password. When you receive your username and password, just go to the Employer Tools section to begin using these valuable tools.
