Mail-order prescriptions
Our mail-order pharmacy offers different ways to order prescription refills.
For commercial members:
- Online through the Caremark's Web site.
- By phone: access the automated refill system by calling 1-888-543-5919 (toll-free)
- By mail: included with each order will be a pre-addressed, postage-paid envelope which can be used to refill or order a new prescription
- By fax: physicians can fax in new or refill orders to this fax number: 1-800-378-0323
For Fallon Senior Plan members:
- Online through the Caremark's Web site.
- By calling toll-free at 1-800-311-0572 (TDD/TTY: 1-866-236-1069), to access an automated refill system.
- By mail:

- Physicians can fax in new or refill orders to 1-800-378-0323.
Prescriptions will be delivered free to the home within 10 to 14 days from when the prescription or refill request is received.
Placing a mail-order prescription for the first time

- Call in the prescription directly to the mail-order pharmacy by calling 1-800-378-5697, option 2, or
- Fax it to 1-800-378-0323
- Mail in the prescription to:

If you use the online service, be sure to sign up for the mail-order pharmacy’s refill reminder program at www.caremark.com. With this service, you’ll get convenient e-mail notices from the mail-order pharmacy when it’s time to refill or renew a prescription.
