Register for Quick Quote

Are you currently an FCHP broker?

If you're already an FCHP broker, registering for Quick Quote is easy!

1. Download the Quick Quote registration form (pdf)
2. Fax or mail the completed form to FCHP.
3. After we've received and processed your form, we'll contact you with your username and password for Quick Quote. When you're ready to start using the tool, just return to the Broker's section on www.fchp.org and log in!

Are you new to FCHP?

To begin selling FCHP products using Quick Quote, first you'll need to enroll with FCHP as a broker.

1. Download our New Broker Enrollment Kit (pdf)
   OR
   Contact us at 1-800-333-2535, ext. 79097 or by e-mail at broker@fchp.org to request information.
2. Fax or mail your completed enrollment kit and a copy of your broker license to FCHP.
3. After we've received and reviewed your materials, we'll contact you with all the information you'll to start selling FCHP products.
