How to find the plan type and prescription drug copayments**
This information is listed on the FCHP membership ID card.

- Plan type is at the top of the card.
- A "Y" after the "Rx" label indicates that the member has FCHP prescription coverage. An "N" means that there is no prescription coverage through FCHP.
- The numbers after "COPAY" are the copayment amounts for Tiers 1, 2 and 3.
